ADOPT-A-BLOCK FEEDBACK

Thank you for your participation in the Adopt-A-Block program this year! 

Please take the time to fill out this feedback form.  We are excited to hear how your year went, and learn what was valuable to your block.  Thank you for taking the time to be thoughtful in your responses.

Twenty-five blocks who submit the most thorough feedback forms will receive a $10 gift certificate to Kroger!
